After talking with my preceptor we determined the following aspects in my change proposal.
Financial Aspect: A initial training will need to be given to all RN’s who will implement the Initial RN assessment. Another financial concern was thinking if this new project should be held by just one RN and if so would that require a new hire? When looking at the bigger picture, this project is aimed at decreasing the number of future calls and greater number of office visits. So that could then help the organization financially.
Quality Aspect: The idea behind my project is to better improve the patient quality. So if it is implemented, patients will be approached with a patient-centered model and that has proved to improve the quality of care and patient outcomes.
Clinical Aspect: Clinically, the workflow will need to be changed and a new questionnaire will need to be initiated by the RN. I have thought about ways to have the questionnaire completed and one way could be using the patient ipads they already use in this clinic to check in. The questionnaire can be preloaded, the patient completes prior to seeing the RN. My ultimate request would be to have the RN complete the questionnaire with the patient in person so that it is used as a moment for communication and building trust.
